Warranty and Returns Policy
Mountainview Prosthetics Inc. will warrant all prosthesis for workmanship for the period of 120 days. Components supplied by Mountainview Prosthetics Inc are limited to manufacturer’s warranty. Modifications done by Mountainview Prosthetics Inc are only guaranteed for fit if the job is accompanied by a completed measurement form and work order. Please see applicable work order for required measurements. Warranty covers defects in material and workmanship only. Test sockets and partial assemblies are not covered. Warranty does not cover damage due to accidents, misuse, tampering, alterations or improper use.

In-House Fab Time & Rush Orders
Normal turnaround time once an order is received at Mountainview Prosthetics Inc. is 24 hours for test socket 2-4 days for finishing work and laminations. We reserve the right to add a rush charge for prosthetics if a turnaround time of 24 hours or less is required (except test sockets). Mountainview Prosthetics Inc is not responsible for delays occurring if proper work order is not received.

Components
All prosthesis fabricated will utilize customer requested components. Components not included in price.

Special Orders
Special orders (non stock) components cannot be returned, unless manufacture defect. Mountainview Prosthetics Inc will special order components with proper work order from customer. Customer responsible for cost plus shipping of all special order items.

Service Charges
Past due accounts are subject to a 1½% per month service charge, with an effective annual rate of 18%.

Payment Terms
Payment terms are 2% 15 DAYS & NET 30 DAYS from the date of invoice, unless already agreed upon with customer.

Past Due Accounts
Accounts with balances past 60 days will be shipped COD. We reserve the right to suspend shipments if any portion of an account becomes past 90 days, unless satisfactory arrangements have been made with the Accounting Department.

Necessary Information
All orders must be accompanied by a completed work order. We reserve the right to hold work until properly executed work order is received.
